Why refrigerated mail shipments fail: five failure modes
Refrigerated mail shipments fail in five recurring ways: summer heat outlasting the packout’s hold time; winter ambient freezing the product; the right packout run in the wrong season for the lane; weekend or holiday holdover stretching transit past the rated hold; and refrigerant conditioning errors that freeze the product from inside the box. Every one is a known, designable-against mode — none requires bad luck, only a packout program that ignores it.
The failures are also mostly invisible. A heat-stressed vial and a frozen-and-thawed pen both look normal on the doorstep, which is why the failure you hear about is a fraction of the failure rate you have.
Mail-order pharmacy moves an enormous share of American prescriptions with remarkably little independent scrutiny of the thermal ride. When NBC News investigated the system in December 2020, its conclusion was not that packages fail occasionally — it was that almost nobody is positioned to know when they do.
Oversight of shipping to patients — during which drugs might be exposed to heat waves and below-freezing temperatures — is largely a system of blind trust.

Blind trust is not a packaging strategy. Here are the five ways the trust actually breaks, and the fix for each.
Mode 1: Summer heat outlasts the packout
The textbook failure. The packout was built for the happy path — a 36-hour lane — and the shipment took 60 hours because a sort was missed, a plane was weather-held, or the first delivery attempt failed. The refrigerant exhausted its capacity somewhere in hour 40, and the payload spent the remainder equilibrating toward a July trailer. The ambient baseline is worse than intuition suggests. A 2023 study in the Journal of the American Pharmacists Association shipped 36 logger-equipped parcels between New Jersey, California, and Tennessee across three carriers in both winter and summer, and found 68.3% of transit time was spent outside USP’s controlled room temperature range regardless of carrier, method, or season (Chowdhury et al., JAPhA, 2023). That is the environment your refrigerated payload rides through.
The fix: rate the packout against worst-case door-to-door time — carrier delay days plus porch dwell — not the service’s published estimate, and validate that hold with a summer-peak chamber profile. The tier arithmetic is in choosing a packout hold time.
Mode 2: Winter freezes the product
Winter inverts the problem, and it is the less forgiving direction. GLP-1 and insulin labeling treats frozen product as an unconditional discard — Wegovy’s label says “do not use if it has been frozen,” Mounjaro’s says “do not use Mounjaro if frozen,” and Lantus’s says “discard Lantus if it has been frozen” (Novo Nordisk; Eli Lilly; Sanofi labeling, 2026). There is no re-qualification path, and a thawed pen shows nothing. NBC News’ 2020 investigation flagged below-freezing exposure as one of the two hazards mail-order patients cannot see; its 2022 follow-up found that most state pharmacy boards had no specific rules for how pharmacies ship medication at all, and that Oklahoma’s 2022 proposal to control shipping temperatures was the first detailed rule of its kind in the nation (NBC News, 2022) — one that promptly drew industry opposition (NBC News, 2023).
The fix: a winter packout variant designed around the freeze line first — reduced refrigerant mass, warmer conditioning, and insulation working to keep cold out rather than in. The mechanics are in the freeze problem.
Mode 3: The right packout in the wrong season
Most programs that own both a summer and a winter configuration still fail in the seams, because they switch on a single calendar date for every lane. Climate does not cooperate: a Phoenix-bound lane in October is still a summer lane, a Denver-bound lane the same week may already be a freeze risk, and a shoulder-season shipment can plausibly see both hazards in one transit. A packout that is correct in aggregate is still wrong on the lane in front of it.
The fix: assign configurations by lane and forecast, not by company-wide date — define the trigger conditions (origin, destination, and route temperatures) that flip each lane, and write them into the SOP so the decision is procedural rather than judgment-by-packer. Mode 4: Weekend and holiday holdover
A parcel that misses its delivery window before a weekend does not keep moving — it sits, in a trailer or depot that is neither heated nor cooled, until the network resumes. A Thursday shipment on a two-day lane that slips one day arrives Monday: that is roughly 96 hours door to door, which exceeds every rated hold Meridian makes — and most that anyone makes. Holiday weeks are worse, because a slipped Wednesday pickup can strand a box across three non-moving days.
The fix: this one is free. Constrain ship days so that worst-case transit still lands on a business day — for most two-day lanes that means Monday through Wednesday cutoffs — and maintain an embargo calendar for holiday weeks. The full scheduling logic is in the peptide shipping calendar.
Mode 5: Refrigerant conditioning errors
The failure mode that happens entirely inside your own building. A gel pack pulled straight from a −20°C freezer is far colder than the 2°C floor the product is allowed to touch, and if it is packed in direct contact with the primary container, it will freeze the adjacent product while the box average reads a compliant 5°C — a summer shipment can freeze its own payload in July this way. The opposite error is quieter: packs conditioned too long, or packed from a refrigerator when the design assumed a freezer, start the trip partially spent and shave hours off the rated hold.
The fix: treat conditioning as a controlled process step — specified temperature, specified duration, staged location, logged — and never let refrigerant touch primary packaging: the barrier layer in the packout diagram is load-bearing, not padding. The five modes leave different fingerprints. A single-use indicator or logger — available from any monitoring supplier; Meridian makes packouts, not monitors — separates heat from freeze. The tracking file separates transit failures from design failures: a compliant 70-hour scan history points at modes 1 and 4, a Friday ship date at mode 4, a frozen reading in August at mode 5. Without a temperature record, every one of these failures is invisible — which is the “blind trust” NBC News was describing.
Frequently asked questions
What temperature must refrigerated medication stay at during shipping?
2–8°C, the labeled storage condition for refrigerated product. USP <659> tolerates transport excursions between 2°C and 15°C for up to 24 hours when mean kinetic temperature stays at or below 8°C — but no excursion below 2°C is permitted, which is why freezing dominates winter design.
Is winter actually riskier than summer for refrigerated shipping?
It is less forgiving, even if it is not more frequent. A hot arrival may still be adjudicated compliant under the USP <659> excursion envelope; a frozen arrival is an unconditional discard under GLP-1 and insulin labeling, with no analysis to run. Winter also adds the self-inflicted variant: over-cold refrigerant freezing the product inside an otherwise sound packout.
How long can a refrigerated shipment survive a carrier delay?
Exactly as long as its rated hold minus elapsed time, which is why the rating must be chosen against worst-case transit rather than the published estimate. A 72-hour packout on a two-day lane leaves roughly a day of real margin; a 48-hour packout on the same lane leaves nearly none once porch dwell is counted.
Should gel packs be frozen or refrigerated before packing?
Whatever the validated packout design specifies — that is the point. Frozen packs deliver more hold but must be conditioned (tempered toward their working temperature, for a specified time) and separated from the product, or they become a freeze source. Refrigerated packs are safer against freezing but hold less. Substituting one for the other invalidates the design.
Who regulates the temperature of mailed prescriptions?
State boards of pharmacy, mostly — and thinly. NBC News’ reporting found most boards had no specific shipping-temperature rules as of 2022, with Oklahoma’s proposed rule the first detailed attempt in the nation.
